Is it possible to insert explanatory text fields into a User UI queue screen?

One department wants a queue, but they want to insert explanatory text onto the User UI screen near the appropriate fields on the form. For example:

If outside services are needed, whose budget will pay for it?

This includes printing, translation, mailing, or other external vendor services.

Yes use custom fields set to read only and added to the template.

If you want to change the text in the same location you may need to use a ticket rule to update the text based on a selection in the ticket.
